WHAT IS HYPERTHYORIDISM AND ITS INCIDENCE RATE?


Hyperthyroidism is caused by excessive secretion of thyroid hormones. The prevalence of hyperthyroidism in our country is uncertain, but probably around 1-2 %. The disease occurs in all age groups, but is especially common in people between 20 and 40 years. The incidence is 4-5 times higher in women than in men. Autoimmune hyperthyroidism (Graves ‘ disease or Basedow’s disease) is the most common cause in our country (representing 70-80 % of cases).


WHAT ARE THE CAUSES OF HYPERTHYRODISM?


Autonomous nodules in the thyroid gland are another cause of hyperthyroidism. They may be monofocal (toxic adenoma, ca. 5 %) or multifocal (toxic nodular goiter, ca. 10 %). Autonomous nodules are most common in middle-aged and older persons. Thyroiditis can cause thyrotoxicosis, which is usually short-lived, and resolve without management (representing 10.5 % of the cases). This applies to both autoimmune, drugs triggered Thyroiditis and subacute Thyroiditis. The autoimmune hyperthyroidism is caused by the production of TSH receptor autoantibodies (TRAS) that bind to TSH receptors and stimulates follicular cells to increased secretion and cell division. Toxic adenomas may be due to activating mutations in the TSH receptor. The mutant TSH receptor stimulates follicular cells continuously (constitutive activation), regardless of TSH binding and TSH activation. Hyperthyroidism in Hashimoto’s Thyroiditis is caused by hormone release from the destroyed cells.


WHAT ARE THE SYMPTOMS OF HYPERTHYRODISM?


Common symptoms of thyrotoxicosis are heat intolerance , sweating , weakness , weight loss , irritability, agitation , insomnia , tremor , dyspnoea , palpitations , menstrual disorders . In elderly people, the disease can be a symptom of poor and dominated by a main symptom such as weight loss, edema or atrial fibrillation with or without heart failure.


In pregnant women, symptoms can be difficult to interpret, as they are often easily confused with symptoms that occur in pregnancy. HCG stimulation of the thyroid gland can occur as a physiological effect of HCG. Hyperthyroidism will go often rapidly without management. HCG stimulation can cause interpretation problems of laboratory tests during pregnancy.
